The Grimace Shake was a joint venture with McDonald's to try and show love to one of their abandoned characters. In this case, Grimace gets attention for the first time in years. However, Grimace, who was still withdrawing from his heroin addiction that he picked up after McDonald's dropped him in favor of a sentient happy meal, had other plans and beat up everyone who drank it - in other words, he was mean to the friendly Humans. Duh- uh- he heard about the Grimace Shake, and knew that the Humans were not very smart.
When you drink this shake, it will taste good but it will also alert Grimace to your location. If he decides to go after you, he tracks you down using the milkshake inside you (he has extrasensory milkshake perception for some reason) and he will beat you up with all four of his arms. Since normal beat-ups have only two arms, this means Grimace's beat-up is two-and-a-half times as bad as a normal one (the extra 50% is because he's literally a taste bud). He'll also pour your milkshake all over you just to be a jerk.
From the very few who survived, it tasted like berries.

On the obvious and sometimes YouTube Sharts kids will buy the shake, wish Grimace a happy birthday and drink it to experience the side effects. This results in them being mauled by Grimace.
